Co-operative Education Program Student Application
(please ensure letters are at least 3 paragraphs long)		 Complete Online Printable Copy
Student work term forms can be found on Blackboard. Please submit through the Blackboard Drop Box. Be sure to copy and paste into a word document prior to submitting and save the document for your records so that you have a printable version in case the submission is not successful. 

 

 

Employer Forms:

                                                 
Employer Assessment - due last day of placement
 Complete Online

Students must recieve a minimum overall Employer Evaluation grade of satisfactory to recieve credit for their Co-op Work Term. Please contact the Co-op office - 403-220-3580 if you have any questions or concerns about the evaluation. The Co-op Coordinators will contact employers if a less than satisfactory eveluation is recieved.
